A nurse practitioner may serve as the medical director of a New Hampshire med spa and may not operate the laser. That is an unusual combination, and it is exactly the kind of detail a national template gets wrong.
What it does to the economics
Device revenue is modelled on utilisation, and utilisation is modelled on staff availability. If your laser can only be run by a physician or a PA, then:
- Laser hours are capped by physician or PA hours, not by clinic hours.
- The payback period on the device stretches accordingly.
- Hiring an experienced RN injector does not add laser capacity.
- A second location does not double laser throughput unless it doubles physician or PA cover.
How wide the variation is
This is the clearest illustration of why a national laser policy does not exist. Iowa recognises a qualified laser technician category. Ohio allows delegation after 15 observed and 20 supervised procedures per device, with the physician in the room. Maryland treats laser hair removal as a surgical act. Virginia allows a properly trained person under readily-available supervision.

The practical instruction
Before buying a device for a New Hampshire location, write down who will operate it and check their licence against that list. Then model the revenue against those people’s actual hours — not against opening hours, and not against a spreadsheet built for another state.
